The applicant must show proof of hardship by submitting at least one of the following documents: 

• Letter of recommendation from a family member, friend, senior placement provider, social worker, etc., detailing the need for financial support to transition into or remain in a senior living community 
• Past due utility, rent, or mortgage bill.
